Sometimes a waypoint moves slightly when set. Another possibility is that one of more minor changes in heading took place while sailing with a fixed TWA. The result might be a difference between planned and actual heading, which may increase the risk of grounding when very close to shore. I have observed both situations in my experience.

Could be.
In my case, i used TWA and, lets say, I had 2mm distance to the cost square. and I hit ground for 1 mm.
I was controling the heading @ 45º and only changed to 46º when there was enougth clearence to pass Cabo da Roca.
